- comments: no signs posted in washrooms instructing employees to wash hands. Must post signs.
36. Thermometers provided & accurate
- comments: noted no thermometers inside refrigeration unit conspicuously posted to monitor the ambient temperature of equipment. Instructed to equip all refrigeration units with accurate and working thermometer.
51. Plumbing installed; proper backflow devices
- comments: hose attached to basement water tank without a visible backflow prevention device. Must install backflow prevention device.

- comments: noted rear one compartment sink in poor repairs with leakage at the side corner (taped with duck tape). Instructed to remove tape, repair and maintain.

- comments: noted wash rooms on premise with garbage receptacles and no lids. Instructed to provide trash cans with lids at all women wash rooms and unisex wash rooms.

- comments: instructed to detail clean and maintain floors along wall base and underneath equipment especially underneath the cooking equipment and rear 3 compartment sink where roaches were found. Must also sanitize affected areas. Instructed to seal all holes and openings on walls and ceiling and where ever needed to prevent hiding places for pest.

- comments: noted heavy dirt and grease accumulation inside, on pipes/conduits and on edges of the exhaust hood and filters above cooking equipment at the rear prep area. Instructed to detail clean and maintain hood, filters and pipes/conduits.
59. Previous priority foundation violation corrected
- comments: noted previous priority foundation violation #38 (rule #6-501. 111) from 9/16/2020 inspection report #2401220 not corrected:noted over 15 live roaches on floor around the grease trap, on walls and 3 compartment sink at the rear food prep/dry storage. Instructed to detail clean and sanitize affected areas, eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority violation #7-42-090. Citation issued.
