- comments: observed no paper towels provided to handwashing sink in the restroom area. Instructed to provide paper towels to all handwashing sinks at all times and maintain. Priority foundation violation. 7-38-030(c) citation issued.

- comments: observed out of temperature tcs food items in the front upright cooler: cooked sausages at 50. 9f. Instructed to discard and denature food items. Person-in-charge discarded food items and denatured with bleach. 9 bags of sausages at 10 lbs each. Total cost $300 90 lbs priority violation. 7-38-005 citation issued.

- comments: observed raw chicken thawing at room temperature in 3-compartment sink. Instructed to thaw with approved thawing method, such as under running water at 70. 0f or below.

- comments: observed several cold-holding units in the rear storage area and front kitchen area missing internal thermometers, conspicuously posted to monitor the ambient temperature of the equipment. Instructed to equip all cold-holding units with working and accurate thermometers.

- comments: observed approximately 100-150 rodent droppings in the following areas: pit/wood storage area, behind cooling units and bread carts in rear area, beneath front service counter, beneath cooking equipment in kitchen area, beneath shelves in chemical storage area. Instructed to remove droppings and clean and sanitize affected surfaces and maintain. Priority foundation violation. 7-38-020(a) violation issued.

- comments: observed trapped, dead rodent behind cooling units in rear storage area. Instructed to remove and clean and sanitize affected area and maintain.

- comments: observed non food-contact surfaces of all cooking equipment in the kitchen area to be soiled wtih debris and food residue. Instructed to clean and sanitize all non food-contact surfaces at adequate frequency to prevent debris build up and maintain.

- comments: observed grease trap beneath 3-compartment sink heavily soiled and rusted. Instructed to replace with model that is smooth and easily-cleanable and maintain.

- comments: outside waste receptacle/grease disposal area not properly maintained. Refuse and returnables found outside of the receptacle and surrounding area. Grease and other food soil found on ground near grease disposal container. Instructed to remove all waste and soil from the area, clean and maintain. Priority foundation violation. 7-38-020(b) citation issued.

Instructed to clean floors at adequate frequency to prevent buildup of soil and debris and maintain.
