- comments: no soap was provided at both the handwashing sink in the kitchen and the front tray line serving area with opened food. After my request manager provided soap. Priority foundation violation: 7-38-030(c), citation issued

- comments: observed over 60 mice droppings in basement dry storage area under and on top shelves(no gnawed food), also second floor storage area. Instructed to remove droppings, clean and sanitize area. Priority foundation violation: 7-38-020(a), citation issued

Two-door reach-in cooler at storage hallway by the exit door missing interior liner cover for right side of the door, exposed insulation. Repair/replace. The surface must be smooth, cleanable, and non-absorbent material.

- comments: must not use cardboard as a liner for shelving in the pizza prep. Must be smooth and cleanable surface. Accumulated grease and food debris on utility connections and equipment surfaces behind the cooking line. Also grease at interior and exterior of cooking equipment. Must clean and maintain.

- comments: observed in the kitchen a food strainer under three-compartment sink at floor drain to catch all food particles. Instructed to remove food strainer and provide a floor drain strainer.

- comments: cover is needed at receptacles in women's washrooms, must be provided.
