I went to Panera Bread on 11/30, around 2 pm. I had a cup of Chicken Noodle Soup, a half Green Goddess Cobb Salad with Chicken (iceberg was substituted for romaine due to E Coli outbreak), and a baguette on the side. I had no symptoms whatsoever... See More prior to my visit and even exercised in the morning with no complications. This was my first meal of the day. Around 6 pm I began experiencing a headache and left the restaurant early even though I had planned on staying longer to work on school work. Over the next 2 hours, I began to experience severe stomach pains, belching, and my headache intensified. Eventually, I began to experience weakness, chills, mild fever and dizziness. Around 9:30 pm I vomited close 1 gallon worth. Around 10 pm I vomited again, slightly less than the original amount. Shortly after I experienced watery diarrhea. By this point, I was extremely weak, cold, and my body was aching. I had finally stopped vomiting and was able to go to sleep. The following Saturday morning I experienced dry heaving, but no vomit, and had a small amount of diarrhea. I was still very fatigued, and still had body ache and slept most of the day, yet was still tired enough to fall asleep at bedtime. I also had no appetite throughout the day. The following Sunday I had diarrhea one last time and seemed to regain some of my strength. My appetite slightly increased, but not by much. See More in young children, frail or elderly people, and others with weakened immune systems. The recalled Enoki Mushroom was distributed nationwide to distributors and retail stores. No illnesses have been reported to date in connection with this problem.

The Enoki Mushroom comes in a 200g/7.05oz clear plastic package with the following description “Enoki Mushroom” in the front and Green Day Produce, Inc. in the back. The UPC is 16430-69080 and is located on the back side of the package. There is no lot code or dates on the package.
