- comments: observed no chicago food sanitation manager on site or valid certificate posted. A chicago food sanitation manager must be on site and valid certificate posted at all times tcs food prepared and served. Priority foundation 7-38-012

- comments: observed timed faucet not functioning properly at hand washing sink in pre-k washroom. Water must run continously for at least 15 seconds before reactivating faucet.

- comments: observed over 30 mice droppings scattered on floor along walls and corners in gym office, maintenace room behind gym office, inside kitchenette in teachers lounge. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed 1/4' gap on bottom of northeast exit door in gym and west exit door in main building. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed single service articles stored on floor in second floor storage areas. Instructed manager to elevate all single service articles six inches off floor.

- comments: observed rust deposits on grease trap and rolling cart with can opener in prep area. Instructed manager to repair/replace and maintain.

- comments: observed no covered waste receptacle in girls washroom in main building. Instructed manager to provide.

- comments: observed openings in walls in kitchen office, along wall base near freezer in second floor storage area, around pipe fittings under hand washing sinks in washrooms. Instructed manager to seal all openings in walls and ceilings, around pipe fittings and along baseboards in all areas to prevent pest harborage.
55. Physical facilities installed, maintained & clean
- comments: observed unused articles and equipment in rear maintenance room near gym office. Instructed manager to remove all unnecessary items and store equipment six inches off floor and away from walls to prevent pest harborage.
