    This year, 2024, we have seen a surge of illness reports characterized by common symptoms such as diarrhea, stomach pain, and nausea linked to Miracle Whip. Additionally, we have received reports citing strange smell, taste, and unusual color in the Miracle Whip. As of March 25th, 2024,... See More we have received 37 reports linked to any of these issues after consuming/purchasing the product.

    "...there’s a yellow chunky sludge on the inside of the seal and a brownish crust around the edge, and it smells funky"

    "I just bought some miracle whip today. Expiration date is June of this year. I had one already here that I had an open and it’s got the same expiration date. They both smell like paint I got a little sick cause I ate off of one and I had a nasty taste like a, some paint thinner or something"

    "Kraft miracle whip. Hard yellow crusty gunk under the seal. Smelled old. I have 3 jars all with the same expiration date May."

    - comments: 6-201. 11-- observed badly stained, grease encrusted, dirty and missing ceiling tiles throughout the kitchen, above the pizza ovens, must replace. ---- wall fan above the rear kitchen exit door to walkway with excessive dust/grease. ----floors throughout both bars with debris. Must clean and maintain all. 6-201. 17--- noted ceiling light shields badly stained and dirty. Must clean and maintain. ----door of the employee washroom in extremely poor repair with chipping, peeling door material and the bottom portion. Must replace. 6-501. 114--- all clutter, unused equipment, unnecessary stored items on the rear walkway must be removed from the premises to prevent rodent harboring.

    I walk in and order a pizza with cauliflower crust. They pulled it out from under the shelf already premade and maybe prepackaged, not sealed possibly, for who knows how long.     - comments: upon arrival, observed no certified food manager on site, while open and operating. Instructed a city of chicago certified food manager must be on site at all times while open and operating.... See More Priority foundation violation 7-38-012, citation issued

    - comments: observed low-temperature dishwashing machine in use not properly sanitizing dishware. Strength of sanitizer (chlorine) at the time of inspection was 0 ppm. Employee on site was able to repair dishwashing machine. Instructed to monitor equipment ensure it is properly sanitizing at all times and maintain. Priority violation 7-38-025, citation issued

    - comments: observed evidence of pest activity. Observed approximately 35-40 mice droppings on site. The following areas were observed with mice droppings on the floors: under the three compartment sink in the main food prep area surrounding the dishwashing chemicals, in the rear food storage area behind the boxes of vegetable oil. On the floors in the prep area in the basement in the following areas: near the pipes above the grease trap next to the three compartment sink, in the corner near the metal shelving unit across from the dry ingredient containers, in the mop sink closet, on the side of the walk-in cooler, under the black shelving units across from the walk-in freezer, surrounding the equipment in the office, in the alcohol storage closet, surrounding the orange bins stored against the wall near the stairs leading upstairs to the bar. Instructed to eliminate evidence of pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation violation 7-38-020(a), citation issued

    - comments: observed a pair of tongs stored on a roll of paper towel above the handwashing sink in the main food prep/dishwashing area. Instructed to remove said tongs and maintain.

    - comments: observed both the kitchen prep and bar hand washing sinks not accessible for use. Sinks with pots, pans, food strainers stored inside the sinks. Instructed all hand washing sinks must be accessible for use at all times. Priority violation 7-38-030(c) citation issued.

    - comments: noted foods not protected during storage. Observed raw animal origin chicken being stored in the walk-in cooler directly above fresh onions, raw eggs stored directly above open cases of fresh vegetables, raw chicken being stored in the walk-in cooler on the top shelving above assorted prepared foods. All foods removed. Reviewed proper food protection from contamination. Priority violation 7-38-005 citation issued.

    - comments: noted wiping cloths improperly stored throughout the cooks line. Must properly store in a container with a sanitizing solution between use.

    - comments: noted dirty food dishes from the previous days service piled in the bus pans on a bus cart in the server station. Must clean all dirty dishwares timely on the day of service to prevent pest infestation.

    - comments: all stored items throughout the 'office' storage room must be elevated and organized from the floors.

    - comments: noted extremely greasy hot and cold water handles on all hand washing sinks. Must clean and maintain. ----metal rack shelving next to the cooks line and all stored items on the shelving encrusted with grease. Must clean and maintain. ----lower metal shelving of prep table next to the dish machine with excessive rust. Must remove rust, repaint/re-seal.

    - comments: noted the hot and cold handles reversed on the cooks line hand washing sink. Must repair to indicate the proper hot handle for water and cold handle for cold water. -----noted extremely low water pressure of hot water at the mens washroom washbowl. Must provide great water flow.

    - comments: wall at the kitchen hand washing and one compartment sinks extremely dirty/greasy. Must clean and maintain. ----noted the metal floor at the entrance to the walk-in freezer inside the walk-in cooler in extremely poor repair with cracks, missing and damaged metal. Must repair to be smooth and cleanable.

Manager voluntarily discards and denatures all said tcs foods, approx 100 lbs $150. Instructed to maintain a minimum cold holding temperature of 41f. Priority violation. 7-38-005. Citation issued.

    33. Proper cooling methods used; adequate equipment for temperature control
    - comments: observed reach in cooler located on north wall in rear food prep, at an improper cold holding temperature of 50f at time of inspection. Reach in cooler tagged and held for inspection by the chicago department of public health. Instructed to discard all tcs foods and not use reach in cooler. Until it maintains a cold holding temperature of 41f or below and is re-inspected by the chicago department of public health. Priority violation. 7-38-005. Citation issued.

    - comments: found approx. 5 live roaches in rear food prep area behind reach in cooler. Also noted 2 live roaches in basement food prep area next to standing frrzer. Also noted approx 6 small flies located in dry storage area. And 4 small flies located in front prep area/ dining area. Instructed manager to minimize all fly activity. Recommended contacting pest control. Priority foundation violation. 7-38-020(a). Citation issued.

    - comments: found approx 80 dead roaches on site. Observed dead roaches under high temperature dishmaine behind all coolers and freezer and along wall against hood. Must clean and maintain said area.
