5. Procedures for responding to vomiting and diarrheal events
- comments: observed no kit and procedure on site for responding to vomit and diarrhea. Instructed to maintain a written procedure and a kit with all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. No citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towels at hand washing sink located in rear prep area near large cooking pots. Instructed to maintain paper towel at all hand wash sinks at all times. Priority foundation violation 7-38-30(c). No citation issued.
38. Insects, rodents, & animals not present
- comments: observed live roaches, fruit flies, ants, and a spider throughout the facility. Observed live roaches in the following areas: 6 infant roaches behind low temperature dish machine located in rear dish washing area, 12 adult roaches in the interior of low temperature dish machine located in rear dish washing area, 1 live roach by water heater in basement, 3 live roaches in basement pastry area and near cooking oil containers, 3 live roaches underneath flat top on main prep line. Additionally observed fruit flies in the following areas: 30 fruit flies in bar, observed 20 fruit flies in kitchen prep area, observed 10 fruit flies in dish pit area, and several fruit flies in dining area. Also observed live ants in dish pit area and in rear exit hallway located in basement. Instructed to deep clean, contact pest control for additional service, and maintain facility free of pests. Priority violation 7-38-020(a). No citation issued.

- comments: observed torn gaskets on the interior of the prep coolers located on the main prep line on the first floor and in the downstairs prep area. Instructed to repair or replace and maintain in good working order.

- comments: observed cutting surfaces in rear prep area covered in deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable cutting surfaces.

- comments: observed leaks coming from pipes attached to dish machine located in rear dish washing area, white pvc pipe above water heater in basement, and hvac system located near entrance to alcohol cooler located in basement.

- comments: observed heavy build up of grime and dirt on floor behind bar located near entrance to facility. Instructed to clean and maintain.

- comments: observed heavy build up of food debris, grease, grime, and filth underneath, behind, on top of, and above all kitchen equipment and coolers. Instructed to pull all equipment out and deep clean all areas of facility.

- comments: observed no sign indicating that smoking is not allowed on premises. Instructed to install a sign indicating that smoking is not allowed on site. City ordinance violation 7-32-020(a). No citation issued.
