- comments: found the right washbowl in both of the girl's 1st fl toilet room with no running water. Instructed to provide water and maintain.

- comments: found door #3 not rodent/insect proofed. 1/4" gap underneath and 1" gap in between double doors. Instructed to rodent/insect proof doors and maintain.

- comments: found rodent activity on the premises. 40 mice droppings under the ramp at door #3 and 100 mice droppings in the janitorial closet. Also, found potential nestings under the ramp. Instructed to remove droppings/nests, clean/sanitize affected areas and maintain all. Priority foundation 7-38-020(a) no citation issued

- comments: found the floors by door #3 and in the janitor's storage area with leaves, dirt and debris. Instructed to clean and maintain floors. -found tissue on the walls/ceiling in the girls 2nd fl by classroom #206. Instructed to clean and maintain walls/ceiling.
