Symptoms: Diarrhea Fever Nausea Vomiting

I had them last night 3/18/18 around 9:30 pm on my break. This morning I started to feel bad pain in my stomach at work. A co-worker said I got very pale and should go home. But before that I tried to force my self to the bathroom. I had a bit of diarrhea at work. I was sent home. I went to lay down and felt body chills. My head felt a bit warm. I also vomited once so far but still feel nauseous as of now 2:12 pm with huge stomach pains. I can’t wait for this pain to end.

- comments: observed facility with use of the sample clean up policy provided from cdph website and no supplies. Must ensure the availability of both a clean up policy and adequate supplies (disposable mop, disposable latex gloves, disposable mask, and adequate sanitizer *must eliminate norovirus*) on site to properly clean up any vomit or diarrhea events that can potentially occur inside of the food establishment. Priority foundation violation #7-38-005. No citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the following sinks not maintained with hot water: improper temperature of hot water observed at rear prep sink (66-77f) and washroom handsink (61f). Instructed person in charge hot water at all hand washing sinks must be at least 100f. Priority violation #7-38-030(c). Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed facility equipment for cold holding in disrepair. Noted reach in prep cooler (located directly in front of hot-hold station) to be at improper at 51f. Uncooked cheese pizza and pepperoni pizza were removed from unit. Informed person in charge to repair or replace for proper cold holding of 41 degrees f or below. May not be used until reinspected by chicago department of health (cdph). Tagged unit. Priority violation #7-38-005. Citation issued.

36. Thermometers provided & accurate
- comments: observed no ambient air thermometers inside the reach in prep coolers. Instructed to provide conspicuous and easily readable thermometers inside all refrigerator units to properly monitor the ambient air temperature of equipment.

- comments: observed no covered receptacle (waste can) for proper disposal of waste in unisex employee washroom. Instructed to provide covered waste receptacle.

- Dole™ Sunflower Crunch Chopped Salad Kit. UPC: 0-71430-00034-2. Best If Used by Date: 02-11-21. Lot code: W02702A | W02702B.

This recall is due to possible undeclared allergens (wheat and tree nuts) in Dole™ Sunflower Crunch Chopped Salad Kit. The dressing and topping kit (master pack) designed for a different Dole product was unintentionally used during a portion of the production of the salad. Dole Fresh Vegetables, Inc. is coordinating closely with regulatory officials. No illnesses nor allergic reactions have been reported to date in association with the recall. However, people who have an allergy to wheat and tree nuts may have a serious or life-threatening allergic reaction if they consume this product.

The product lot code and Best if Used by Date are located on the upper right corner of each bag and the UPC code is located on the bottom right corner of the back of each bag. The impacted products were distributed in OH, NY and WI. Consumers are advised to check product they have in their homes and discard any product matching the production description, UPC codes, product lot code and Best if Used by Date listed above.

No other Dole products are included in the recall. Only Dole™ Sunflower Crunch Chopped Salad Kit with the exact UPC code, lot code and Best if Used by Date identified above are included in the recall. Consumers who have any remaining product should not consume it, but rather discard it.
