- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

- comments: observed no date markings on tcs ready to eat foods being held for more than 24 hours inside of cooler. Must provide and maintain. (priority foundation 7-38-005)
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 20 or more live roaches crawling in gaps in wall under front prep area hand sink,on walls,under upper ledge of 3- compartment sink,door gasket of 2- door reach in freezer,on walls in between pipes at 3- comp sink,mop sink,electrical panels,wall outlets,stair case leading to basement. Also. Appx. 25 or more live large flies flying around prep,dish washing areas and sales floor. Appx. 15 or more rat droppings on floor in basement behind shelving unit,on floor in between pallets. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 8/2/820. (priority foundation 7-38-020(a)

- comments: observed an appx. '1/2-3/4' gap along bottom of front entry and exit doors. Must make doors tight fitting.

- comments: must store wiping cloths in a sanitizing solution when not in use and not on cutting boards.

- comments: must repair or replace worn door gaskets on 2 & 3- door reach in freezers.

- comments: observed large pots used for cooking carnitas and peppers fro salsa that are too large to be submerged in the 3- compartment sink. Instructed manager to provide equipment that can be fully submerged into sink or have a written procedure for alternative manuel warewashing.

- comments: must clean debris build up from undersides of prep tables,grinder,walk in cooler-shelving units,fan guard covers,pop sleeves,compressor units,interior panel of ice machine.

- comments: observed no hot running water at hand sink in toilet room do to faucet handle missing to turn on water. Must replace missing faucet handle to provide hot running water for hand washing and maintain. (priority 7-38-030(c)
