- comments: observed no employee health policy on site as required; must provide. Instructed the person in charge to provide documentation of employees responsibility to report any illness transmissible through food. Priority foundation 7-38-010

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towels or other hand drying provisions at handsink in rear near 3 compartment or in staff toilet room. Instructed manager to provide at all times. Priority foundation. 7-38-030(c). Citation issued

16. Food-contact surfaces: cleaned & sanitized
- comments: observed black slimy substance build up on inside interior panel of ice machine and dripping onto ice for human consumption. Must remove build up, clean, and sanitize. Priority foundation 7-38-005. Citation issued

36. Thermometers provided & accurate
- comments: observed no ambient air thermometers inside the reach in milk coolers. Instructed to provide conspicuous and easily readable thermometers inside all refrigerator units to properly monitor the ambient air temperature of equipment.

37. Food properly labeled; original container
- comments: observed packaged sandwiches with ingredients, net weight, allergens, business name and address. Instructed manager to properly label all packaged sandwiches.

38. Insects, rodents, & animals not present
- comments: observed the rear exit door not completely rodent proofed as required. Must completely rodent proof door by sealing 1/4 gap at the bottom of the door.

39. Contamination prevented during food preparation, storage & display
- comments: observed a fly swatter and electric insect device positioned over prep cooler near hot holding food units. Instructed manager to remove devices and maintain.

41. Wiping cloths: properly used & stored
- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store in sanitizing solution between uses.

43. In-use utensils: properly stored
- comments: observed ice scoop submerged in ice with shoe strings attached to retrieve handle in ice. Instructed manager to store ice scoop handle up above ice to prevent contamination and remove strings.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no sanitizer test strip kit on-site to test concentration of sanitizer in 3 compartment sink. Instructed to have a test strip kit on site at all times to monitor the strength of the sanitizer pf properly. Priority foundation. 7-38-005 citation issued

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacles in staff toilet room. Instructed to provide.
