2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site while tcs foods are being prepared and served rice, soup, beef etc. Instructed above that certified food manager has to be on site at all times food is being prepared and served. Certified manager arrived during inspection. Priority foundation violation. 7-38-012. No citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed exposed hand washing sink not accessible in food preparation area/dish area observed buckets and stools in front of sink and boxes on top of hand washing sink. Must remove items so that hand washing sink will be accessible for employees to wash their hands. Person in charge removed items during inspection. Must kept hand washing sink accessible at all times priority foundation violation 7-38-030(c) no citation issued.

37. Food properly labeled; original container
- comments: must label food storage containers when food is not in the original container.

38. Insects, rodents, & animals not present
- comments: observed no insect baby cockroach inside soup at this time of inspection however observed two live small cockroaches crawling on floor next to ice machine and on wall behind 2-door tall freezer in food preparation area. Must clean and sanitize areas and must eliminate roach activity. Orkin pest control company last service dated on april 24, 2020. Additional pest control service is needed to eliminate roach activity. Priority foundation violation 7-38-020 no citation issued.

41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored on top of prep tables. Must properly store wiping cloths in a sanitizer solution when not in use.

43. In-use utensils: properly stored
- comments: observed ice scoops store on top of ice machine. Must properly store ice scoops in a clean container to prevent contamination.

55. Physical facilities installed, maintained & clean
- comments: observed dirt and debris on floor throughout basement. Must clean and maintain floor.

56. Adequate ventilation & lighting; designated areas used
- comments: must replace burn out light bulb above prep tables in rear food preparation area.
