- comments: observed two line cooks preparing food without hair restraints. Instructed management that all food handlers must wear a hair restraint, must provide.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed milk crates being used for storage and shelving in the walk-in cooler and throughout the basement storage areas. Must remove all milk crates being for storage and used to elevate food off the floor and provide storage/shelving that is free of unnecessary crevices and designed/constructed to be easily cleanable.

55. Physical facilities installed, maintained & clean
- comments: observed two refrigerators, a keg cooler, and other food related items stored in the outdoor storage shed. Must remove and relocate all food related items into a proper storage area that has been approved for said items.
