- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

- comments: must remove foil lined on shelving units and prep tables.

- comments: must clean debris and grease build up on interior and exterior of fryers.

(a) in addition to the health steps required by executive order no. 2020-43, businesses must take the following additional public health measures to protect their employees, their customers, and all others who come into physical contact with their operations: (3) ensure that every employee who interacts with customers and is able to medically tolerate a mask is wearing a mask for the duration of a shift; observed employee in restaurant without a face covering. Observed the male cashier which was also the cook at front register taking an order and then preparing food,was asked to put a mask on. He continued to prepare food and take orders without putting on mask then mask was placed on face several minutes later. Then employee became irate and pulled off mask screaming and cursing at me. He told me he was closing to get out and turned off the lights. The owner arrived on site afterwards. (citation issued 7-28-060)
