High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee using cell phone on cookline and did not change gloves.

Inspectors found five dead rodents in a storage room away from the dining area. One rodent was found dead in a trap behind a washer, three were dead on the ground behind a dryer and another was dead in a trap near the ice machine.
The rodent violation itself only constitu... ted a warning from the DBPR, but a broken plumbing system at David's was noted as a repeat violation.

“Monday evening (Feb 15th) my fiancé ate at the steakhouse and consumed a variety of meats - chicken, beef, pork. He woke up at 5am Tuesday morning with stomach pains and vomited all the food still in his stomach. Had loose stools and stomach was in knots. Roommate who was with him at the dinner star... ted experiences same symptoms a few hours later; may have had a slight temperature. He had more severe diarrhea - to the point it was just water/liquid. Now Wednesday (Feb 17) and they are both still not recovered. Stomach pains, nausea, no appetite. ”

“We had a son that went into the hospital on 12/28/20 wand we ate it off and on during his stay until discharge 01/01/21. My husband and I ate the cold cut combo sandwich and the sweet onion chicken teriyaki. A couple hours after we eaten yesterday, we both begin to have projectile vomiting and diarr... hea. We went to sacred heart emergency, since I’m pregnant I got 3 liters of fluid, and my husband got 2 liters. We were diagnosed with food poisoning, along with our doctor that said her and her coworker was violently ill after eating there a couple of days before. We also have chills and it feels like a fever from head to toe. ”

“I had a double butter burger with Cheese and and onions and ketchup, mustard and Mayo and fries and a root beer and also a salted Carmel cashew frozen custard yesterday at 12/26/2020 and then I was sick this morning 12/27/2020. I was vomiting profusely and felt nauseated and had very bad diarrhea. I... haven’t been able to eat anything all day and this has ruined my weekend.
