- comments: observed water temperature at hand sinks in north prep area at 66. 7f,toilet room at 69. 8f and mopsink at 67. 8f. Must have hot running water at a minimum of 100. 0f. Must repair and maintain. (priority 7-38-030(c)(citation issued)

- comments: must label food storage containers when food is not in original package.

- comments: observed evidence of appx. 50 or more live small flies,flying around premises,and in basement on walls as well. Could be condusive due to standing black foul smelling waste water in basement hot water /furnance area. Must have a pest control operator to service premises on or after 10/21/21. - comments: must invert multi-use utensils on dish storage shelves.

- comments: must remove rust from lower shelf of preptables.

- comments: must clean grease and food debris build up from fryers,grill table,walk in cooler shelving units.

- comments: observed a large amount of black foul smelling standing sewage water in sub-basement hot water tank/furnance area. Not able to gain entry into area due to the standing water. Must remove standing water repair and maintain. (priority 7-38-030(c)(citation issued)

- comments: observed no garbage can with lid in unisex toilet room. Must provide and maintain.

- comments: must repair or replace damaged floor tiles in hall way outside of toilet room,hole in wall behind door in rear storage area on 2nd fl.
