- comments: observed no hand washing signage at the hand sink at front entrance bar. Must provide.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed inside soda pop gun machine with minor mineral build up at front entrance bar area. Must clean and maintain.
22. Proper cold holding temperatures
- comments: observed raw beef patties, shredded cheese and black bean burgers stored at an improper cold holding temperature in prep cooler at 1st floor kitchen prep area. Raw beef patties were found at a temperature of 46. 4f. Black bean burgers were found at a temperature of 46. 1f. Shredded cheese was found at 44. 2f. Instructed manager cold holding temperature must be at least 41. 0f or lower. Instructed manager to discard and denature food immediately. Food weighs approximately 20lbs at a cost of $25. 00. Priority violation. 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 180 mice droppings throughout establishment in basement. 100 mice droppings behind chest freezer in basement, 30 mice droppings in prep cooler radiator in basement, 20 mice droppings around grease trap area in basement and 30 mice droppings on top of 3 boilers in basement. Instructed to clean and remove all evidence of pest activity. Priority foundation violation. 7-38-020(a). Citation issued.

- comments: observed 2 cooks in main kitchen prep area on first floor without beard restraints. Instructed to wear beard restraints that covers body hair that is designed and worn to effectively keep their hair from contacting exposed food, clean equiptment, utensils and linens.
