Observed disclosure on menu not stating that asterisked animal derived foods are served undercooked. Instructed to update disclosure on menu to state that asterisked animal derived foods are served undercooked or cooked to order. Priority foundation violation 7-38-005 no citation issued

- comments: observed opening at entrance and exit doors of facility. Observed opening along right corner at base of entrance door on kenneth and observed opening at exit door on archer along left side. Instructed to seal all all outer openings to prevent pest entrance.
51. Plumbing installed; proper backflow devices
- comments: unable to locate backflow prevention device on utility sink. Instructed to install backflow prevention device on utility sink so that it shall be located so that it may be serviced and maintained.

- comments: at time of inspection facility provided no proof of allergen awareness training certificates for two employees with city of chicago food service sanitation certificates. Instructed to provide valid proof of allergen training for all employees with city of chicago food service sanitation certificates.
