2 days ago at Del Taco in Ridgecrest. I had the double del meal with a cherry coke. I woke up the next morning and didn’t feel good at all, the feeling hasn’t gone away since. I haven’t been in this much abdominal pain since I had an appendicitis.  | Symptoms: Vomiting, Nausea, Diarrhea

EMPLOYEES SHALL HAVE ADEQUATE KNOWLEDGE OF, AND SHALL BE PROPERLY TRAINED, IN FOOD SAFETY AS IT RELATES TO THEIR ASSIGNED DUTIES.

- OBSERVED SIGNIFICANT LEAKAGE ON THE FLOOR BELOW DRY STORAGE AREA COMING FROM THE ICE MACHINE. ALL LIQUID WASTES MUST BE DISPOSED INTO AN APPROVED DISPOSAL SYSTEM. REPAIR OR REPLACE.

- OBSERVED ROACH INFESTATION THROUGHOUT FACILITY. MULTI GENERATIONAL LIVE ROACHES WERE OBSERVED ON FOOD PREP COUNTERS, BETWEEN CABINETS, ON THE STOVE, AROUND ICE CREAM SERVICE AREA, AND AROUND THE EXTERIOR OF THE WALK IN COOLER. DEAD ROACHES WERE OBSERVED IN DINNING AREA, IN THE WALK IN COOLER, BELOW AND INSIDE CABINETS. FOOD CONTACT SURFACES AND FOOD EQUIPMENT WERE CONTAMINATED. THE FACILITY MUST BE SERVICED BY PEST CONTROL. FACILITY WILL BE CLOSED FOR AT LEAST 48 HOURS AND UNTIL PEST CONTROL DEEMS THE FACILITY CLEAR OF AN ACTIVE INFESTATION PER THEIR SANITARY SURVEY.

- OBSERVED EVIDENCE OF MOUSE DROPPINGS INSIDE THE UNUSED KITCHEN AREA NEAREST THE LARGE DINNING AREA.

- OBSERVED DEAD CRICKETS IN THE LARGE DINNING HALL AREA. THE PREMISES OF THE FACILITY MUST BE MAINTAINED AND OPERATED TO PREVENT THE ENTRANCE OR HARBORING AT ALL TIMES. REPAIR OR REPLACE AND CLEAN ALL DEAD VERMIN.
