- comments: 2-101. 11 observed the person in charge at the time of the re-inspection without a city of chicago food service sanitation manager's certificate while tcs foods (beef) are being prepared
handled and served to the public. Instructed the person in charge must obtain the city of chicago food service sanitation manager's certificate and be on site at all times the facility is opened and operating. Priority foundation citation issued under violation #2.
2. City of chicago food service sanitation certificate
- comments: 2-101. 11 observed no city of chicago food service sanitation manager on site at the time of the re-inspection while tcs foods (beef) are being prepared handled and served to the public. Instructed the city of chicago food service sanitation manager must be on site at all times the facility is opened and operating. Priority foundation. Citation issued 7-38-012.
16. Food-contact surfaces: cleaned & sanitized
- comments: 4-602. 11(b-e) observed a minor brownish substance on 1 of 3 soda nozzles located behind the bar area. Instructed to clean and sanitize.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 upon re-inspection observed evidence of live insect activity on site, observed about 5 large flies flying in the food prep area and in the customer dining area. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).
38. Insects, rodents, & animals not present
- comments: 6-501. 112 observed dead mosquitos and spiders caught in spider webs in various corners of the men's toilet room as well as in the small hallway outside of the men's toilet room and in the utility closet located directly outside of the men's toilet room. Instructed to remove all dead insects and clean and sanitize said areas.
45. Single-use/single-service articles: properly stored & used
- comments: 4-904. 11 observed the customer dining tables being preset without adequate protection for tableware (forks, knives, spoons). Instructed to provide adequate protection for all table utensils to prevent from possible contamination or do not preset customer dining tables or set the customer dining tables upon customer arrival.
51. Plumbing installed; proper backflow devices
- comments: 5-204. 12 unable to locate the back flow prevention device on the ice machine located in the dish washing area. Instructed to locate or provide.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 5-501. 17 observed no covered receptacle inside of the rear employee's toilet room located in the dry storage area. Instructed to provide.
55. Physical facilities installed, maintained & clean
- comments: 6-501. 114 observed items stored directly on the ground in the rear chemical storage room and under the equipment shelves all located in the dish washing area. Instructed to elevate all items at least 6 inches from the ground and maintain.
57. All food employees have food handler training
- comments: 2-102. 13 observed 1 of 2 male food handlers without a food handlers certificate. Instructed to meet requirement for all food handlers within 30 days of hire and maintain.
59. Previous priority foundation violation corrected
- comments: 8-404. 13(b:3) observed previous priority foundation violation not corrected from inspection report #2577314 dated 6/14/23; 25-3-603. 11 observed no consumer advisory disclosure (with foot note) on the menu for all items that can be ordered undercooked or raw (hamburger, surf and turf, ribeye, new york strip and filet mignon items) as previously violated on inspection report #2556616 dated 6/2/22 (grace period given until 8/25/22). Instructed to provide a disclosure (with foot note) on all menus (digital and physical). Priority violation. Citation issued 7-42-090.
61. Summary report displayed and visible to the public
- comments: 8-304. 11(a) upon arrival observed the facility did not have the most recent inspection report summary posted for the public's view and not visible to the public. Employee posted the summary during the re-inspection. Instructed to facility to always have the most recent inspection report summary posted and visible to the public at all times. Priority foundation. Citation issued 7-42-010(b).
