    I brought nachos with cheese and salsa dip and an hour into the movie found myself being sick about 5 times and must of brought up the whole cheese dip.

    Dined there for dinner with my son. He is 35 years old and I am 65 years old. Our dinnerchoises were Udami bowel for me and Civechie nachos for my son. Presentation of both dishes were awesome.
    Unfortunately I became sick with in 3hours. Chills, abdominal cramping... See More and diarrhea. In the morning I could not eat or drink due to abdominal discomfort.
    Drove to Houston airport to catch a flight and due to low grade fever I took one extrastrength Tylenol with O.J..
    Violently vomited up all the food I ate the night before.

    We missed our flight back to Newark, NJ, due to need to be over a toilet to empty my bowels.

    I will reach out to Discover Card and report the incident to the health department.

    - comments: observed manager on site without city of chicago food service... See More managers certification. Insformed and instructed that certified food service manager must be on site at all times tcs foods such as chicken, pork and other meats are being prepared and served. Priority foundation violation 7-38-012 citation issued.

    - comments: observed evidence of pest activity in rear meat prep area under the green metal shelving units apprx 50-55 droppings approx another 5 droppings under shelving between the racks of dish washing liquid and the shelving with the snacks moon pies, cookies etc. Also observed live fly activity of several large flies and smaller flies. Instructed to clean and maintain areas. Instructed to contact pest control and have a service provided along with a pest service report. Priority foundation violation 7-38-020(a) citation issued.

    - comments: observed the inside of the cold holding units (reach in cooler, meat display unit) with a build up of dirt, dust and debris and splatter inside of the walk in cooler. Observed the walk in freezer with a build up of ice, dirt and debris, instructed to clean and maintain all.

    - comments: observed the rear three compartment sink faucet head broken in need of repair. Instructed to repair and maintain in good working condition.

    53. Toilet facilities: properly constructed, supplied, & cleaned
    - comments: observed missing lid on receptacle in the rear employee toilet room. Instructed to obtain receptacle with lid.

    55. Physical facilities installed, maintained & clean
    - comments: observed the floors throughout the facility with a build up of dust and dirt, in the rear three sink area and the rear meat prep area under the shelving areas as well as along the walls on the floors. Instructed to clean and maintain all areas.
