    - comments: observed no disclosure and reminder on the printed menu for all animal food by statement of the person in charge; some products can be ordered raw or undercooked. Instructed to provide a disclosure on the menu for all items (eggs any style, steak, beef burgers etc. ) that can be ordered undercooked or raw, and add a reminder on the menu that reads: *these items can be ordered undercooked or raw. Consuming undercooked or raw foods can increase your risk of foodborne illness. (refer to cdph consumer advisory handout) priority foundation violation #7-38-005.

    - comments: observed food handlers prepping food without hair restraints. Instructed person in charge that all food handlers must wear a hair restraint.

    - comments: observed wet wiping cloths on several prep surface areas. Instructed to properly store wiping cloths in a sanitizing solution between uses to prevent contamination of equipment & food. Must maintain

    - comments: observed drainpipe at dishwash handsink with a small leak. Instructed to repair and maintain.

    55. Physical facilities installed, maintained & clean
    - comments: observed loose debris, dirt and dust buildup on the floors and corners in basement storage area. Instructed to detail clean and increase cleaning frequency of floors. Must maintain at all times.

    60. Previous core violation corrected
    - comments: previous core violation from report #2573312 on 3/28/23 were not corrected: #51- observed broken sensor at hand sink in women's bathroom left side. Must repair and provide hot and cold running water. #53- observed bathroom door in basement not self-closing. Must install self-closing device on door. #58-observed no allergen training for the city of chicago food manager. All city of chicago food managers must complete allergen training and keep on site. Priority foundation 7-42-090. Citation issued.
