- comments: must not use milk crates as a means of elevation for stored items. Must not use tape as padding for the corner of shelves in rear prep area. Must be smooth and cleanable. Presently broken and dirty tape.

- comments: not able to locate back flow prevention device is provided at east ice machine water line, at front serving area. Must install so back flow prevention device may be located to be serviced and maintained or to know the location.

Interior of the ice machines upper water reservoir, with excessive black slime substance dripping down onto the ice and ice bin. Instructed to clean and sanitize the interior of the ice machine throughout. Ice is used for human consumption. Priority foundation violation 7-38-005 citation issued. Observed today ice machine water reservoir still with black slime and not clean. #38)6-501. 111: observed over 200 mice droppings in different locations of premises: kitchen/pizza areas,on shelves by the food and food containers,on prep table by food containers. Under three compartment sink(south-west corner), along baseboard walls. Under plastic crates. Also in storage areas. Observed small flies(about 10) by the three compartment sink on walls also front serving and dining area. Must remove droppings and flies,clean and sanitize area. Last service from pest control was on 3-19-24. Priority foundation violation: 7-38-020(a),citation issued today 4/2/24 still observed mice droppings throughout the premises. Including in dining room under booths, food preparation and storage areas, in boxes with food contact equipment, inside rear employee washroom, and inside walk-in cooler. Over 200 rodent droppings priority violation:7-42-090, citation issued.
