- comments: observed over 75 small, black flies in the following areas: flying around north side of prep area; on walls and ceiling on north side of prep area; on north wall of the dining... See More room, and in restrooms. Instructed must detail clean and sanitize all affected areas. Must contact pest control and minimize pest issue. Priority foundation violation 7-38-020(a). Citation issued.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed heavily soiled utensils (plastic containers, etc. ) in front prep area. Must detail clean and sanitize all utensils.

49. Non-food/food contact surfaces clean
- comments: observed heavily soiled counters (north side) in front prep area. Observed pooliing liquid on counters. Must detail clean and sanitize area. Must keep clean and dry to prevent pest harborage. Must maintain same.

51. Plumbing installed; proper backflow devices
- comments: noted leak from main faucet of front beverage prep sink (hose attached to 1 of 2 sink faucets). Instructed to repair.

- comments: observed liquid and debris accumulating in large gaps between tiles in front prep area, near beverage prep area. Must detail clean and sanitize this area. Must fill gaps (grout) between tiles to prevent liquid pooling. Must keep floors clean and dry. Must make floors even, smooth and easily cleanable. Must maintain same. Observed dust accumulation on north wall of dining room and on walls of restrooms. Must detail clean. Must maintain same.

- comments: observed a food handling employee touching ready-to-eat food with their bare hands during preparation. Observed the food employee handling cooked sliced sausages at prep line... - comments: observed a slight debris accumulation encrusted on the interior of the ice machine. Must remove debris, detail clean inside ice machine/have serviced and maintain ice machine at all times.

- comments: found tcs foods at improper temperatures: leafy cut romaine lettuce at 61. 2f-62. 4f inside the 5 door prep line cooler at front prep area and cooked sliced sausages at 48. 2 f inside the pizza prep line cooler at rear prep area. Instructed manager, (tcs foods) must maintain cold holding temperature of 41. 0f or colder and hot holding temperature of 135 f or hotter at all times. Priority violation 7-38-005, citation issued.

- comments: found the front 5 door prep line cooler not maintaining proper temperature in use with tcs foods (with an air temperature of 62. 2 f).
