Brown rice, black bean, chicken, mild salsa, cheese, lettuce. Vomiting, diarrhea, abdominal cramping that started 10-12 hours after eating.

This is the same location I ate at on 6/13 at 1:30 pm. I was extremely ill that night and am still recovering.
