- comments: observed ready to eat, temperature control for safety foods (bhindi masala, chana masala, dan makhani-all cooked vegetables)not properly dated , stored longer than 24 hours in coolers. Instructed to provide proper date marking with disposition date for rte foods prepared on site and held over 24 hours in cold holding unit. Priority foundation violation. No citation issued. 7-38-005

- comments: observed spray bottle with unknown chemical stored in dishwashing area without proper common name marking. Must provide proper marking on all chemical containers. Priority foundation violation, 7-38-015 no citation issued

- comments: noted container with around 20 lbs of yogurt noted facility is making own yogurt. Haccp plan not on site. Management voluntarily discarded yogurt. Priority foundation #7-38-005 no citation issued

- comments: observed facility not meeting compliance with variance/specialized process in facility noted yogurt making on site with out haccp and variance on site issued by regulatory authority. Must provide and maintain. Priority foundation violation #7-38-005 citation issued

- comments: observed several large food containers with dry products. (flour , salt, sugar) with out proper common food name markings. All bulk and working food ingredient containers must be labeled with contents name. Must label food storage containers when food is not in original package.

- comments: observed 10-15 old rat droppings in basement floors at the basement landlord utility room. Instructed to clean and sanitize affected areas. Recommended to consult with pest control company. Must also eliminate must lean and sanitize affected area and consult pest control company. Priority foundation #7-38-020(a) no citation issued.
44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed dispensing utensils stored in food with out handles. Instructed manager that utensils must have handles and must be properly stored and inverted to prevent contamination before use(handles-up).

- comments: noted ice build up inside the walk in freezer on the floor and shelves. Instructed to repair and maintain the condensation units and door seals
51. Plumbing installed; proper backflow devices
- comments: backflow prevention devices not located at ice machine. Must install backflow prevention devices or air gap between the water supply inlet and the unit so it may be located to be serviced and maintained
55. Physical facilities installed, maintained & clean
- comments: must clean floors in basement area in all rooms attached and opened to food storage
56. Adequate ventilation & lighting; designated areas used
- comments: must repair and replace missing and non working lights in basement walk in cooler and freezer
