- comments: observed cold tcs food stored at improper temperature in coolers, raw chicken 49. 3f, hot dog 49. 6f, ground beef 50. 3f, italian beef 51. 4f, deli meat 51. 6f, turkey 57. 2f, tzatziki sauce 58. 8f-78. 3f, cheesecake 60. 6f, sliced tomatoes 80. 4f. Instructed manager to discard approximaetly 150lbs and $300 of product. All cold tcs food must maintain 41f or below during storage, preparation, display and service. - comments: observed coolers not maintaining proper temperature, prep cooler 81. 3f, front reach-in cooler 58. 1f, rear reach-in cooler 52. 3f. Instructed manager to repair and maintain coolers at 41f or below. Coolers are tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005 citation issued

- comments: observed bulk containers not labeled in rear prep area. Instructed manager to label all bulk containers with common name of food.

- comments: observed over 30 rat droppings scattered on floor along walls and corners near water heater, 20 rat droppings on floor under pop crates in rear storage area, 20 rat droppings on floor along walls and corners near toilet in washroom. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed food items stored on floor in prep and storage areas. Instructed manager to elevate all items six inches off floor.

- comments: observed pop crates used as shelving in rear storage area. Instructed manager to provide adequate shelving for easy cleaning and pest control.

- comments: observed food debris and grease buildup on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units. Instructed manager to clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed excess cardboard along walls in rear storage area. Instructed manager to remove to prevent pest harborage.

- comments: observed no allergen training available for certified manager on duty. All certified managers must provide allergen training.
