- comments: observed no certified food manager on duty while tcs foods are being prepared and handled such as (pizza dough,pizza sauce,etc) must be on site at all times. Manager arrived on site after inspection was conducted and citations written at 12:26 p. M. (priority foundation 7-38-012)(citation issued)

- comments: observed no hand washing signs at hand sinks in prep /dish washing areas and toilet rooms. Must provide and maintain.

- comments: observed evidence of appx. 100 or more mice and rat droppings on floor next to and behind small reach in freezer at call order area,on floor in between 1- door reach in cooler and unused cooler,on pipe behind same cooler,hot water tank area,rear garage area in between articles on floor and along wall bases,under pizza prep counter. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 1/15/21. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap along bottom of garage overhead door. Must make tight fitting.
