On Sunday September 1, 2019 at 9:17pm my spouse and I ate. We both had ice tea and shared a Bruschetta. I ate a California Salad with a shrimp skewer added. My spouse ate a different entree (Chili Chicken bowl). Our server is also the veritable Manager at that location. I have the receipts to verify the meal and time. At approximately 11:45pm - 12:00am I became very ill and started vomiting. This continued up to 11:30am when I was taken to the emergency room at Ajax Pickering Hospital by my spouse and my Mother. I remained at the hospital from approximately 12pm - 7pm. I was assessed, completed bloodwork and was treated by intravenous fluids for dehydration and anti-nausea medication. The treating physician advised that I had food poisoning. I was also provided with a prescription to manage any recurring nausea. My daughter contacted the restaurant on September 2, 2019 and was advised that the General Manager would contact me. I did not receive a call and I called the restaurant myself on September 3, 2019 and I spoke with him, who was also our server on September 1, 2019. He advised me that I would have to provide a sample of the food for it to be tested to verify that I had food poisoning. He said that other people ate the salad and didn't get sick. I advised that we had a late dinner and I did not eat anything else that night. I told him that this should be a concern for other patrons. He did not appear to be very concerned. At my request he provided me with the name and contact email for the General Manager and the contact number for their Head Office, stated that he has given me all the information that he can provide. I did advise him that I intended to escalate the matter. I did not feel Brandon Bruno was very empathic to my situation or illness which was directly related to the meal I ate at their restaurant. I would like to file this as a complaint | Symptoms: Nausea, Vomiting, Dehydration

As of Feb. 6, 2024, the FDA reported receiving adverse events associated with use of Nurse Assist products and is further evaluating this information. Avanos has not been directly contacted with any reported adverse events.

The product comes in a 5 lb., blue paper pack marked Best if Used by 03/07/2025 on the top of the front panel of the bag.

The recall was initiated after it was discovered by the firm that an equipment breakdown during production could have allowed allergen cross-contact.

Consumers who have purchased 5 lb. Food Club All Purpose Flour Best if Used by 03/07/2025 are urged to return them to the place of purchase for a full refund.

Not sure from what restaurants but I was at Deluxe on Lansdowne, Friday supper time, had a cheeseburger. Pretty sure it was the burger. Also had one on Saturday at East Coast burger, Grand Bay Ate the cheeseburger at Eastcoast Burgers, Grand Bay around 3:00pm.  | Symptoms: Vomiting, Nausea, Diarrhea
