- comments: 2-501. 11 observed no written procedure nor adequate supplies on site for vomit and diarrhea clean up. Instructed to provide. Priority foundation. Citation issued 7-38-005.

- comments: 4-602. 11(b-e) observed all food contact surfaces with accumulated debris. Observed the interior of the ice machine with a brownish substance. Instructed to clean all.

- comments: 3-501. 13 observed frozen beef thawing at room temperature. Instructed to thaw tcs foods under cold running water (no greater than 70f) or in the refrigerator and do not thaw foods at room temperatures.

- comments: 6-501. 111 observed evidence of live rodent and live insect activity. Observed about 30 rodent droppings located by the utility sink in the rear food prep area and about 10 live small flies located on the walls in the hallway outside of the toilet rooms. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 4-101. 19 observed vent missing from the condenser located in the walk in cooler. Instructed to replace.

- comments: 4-601. 11(c) observed accumulated grim on the handles and outside of the refrigeration units and in between all cooking equipment, accumulated dust and dirt debris on all of the lower shelves of all prep tables, accumulated grease and food debris on the wok station and faucets and handles of all sinks. Observed accumulated dust on all vents thru out the food prep area. Instructed to clean all.

- comments: 5-501. 15 observed excessive accumulated grease on the lid of the outside grease dumpster. Instructed to clean. Priority foundation. Citation issued 7-38-020(b).

- comments: 6-501. 114 observed excessive clutter thru out the food prep area and items being stored on the ground. Instructed to remove excessive clutter and only store all items at least 6 inches from the ground.

Instructed to remove dust and maintain. Also clean the floor under the wok station, fryer and other cooking equipment. 55 - 6-501. 114 observed clutter in rear storage area, instructed to organize, remove items not being used. 58 - pa 100-0367 observed no allergent training on premises, instructed to provide. Instructed to correct all previous core violations. Priority foundation. Citation issued 7-42-090.
