2. City of chicago food service sanitation certificate
- comments: observed no city of chicago food sanitation manager certificate on site. Observed no city of chicago food sanitation manager on site while tcs foods are being prepped (chicken, eggs, etc. ) instructed must have a valid city of chicago food sanitation certificate posted in plain view of customers at all times. Must have a city of chicago food sanitation manager on site while tcs foods are being prepped and/or served. Priority foundation violation 7-38-012. Citation issude.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no employee health policy on site. Must provide written employee health policy for all employees. Priority foundation violation. 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit and diarrhea clean up kit/procedure on site. Must provide written procedure for the clean up of vomit and diarrheal events, and corresponding equipment outline in procedure. Priority foundation violation 7-38-005.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed 0ppm of sanitizer available inside low temperature dish machine (in use). Must provide adequate means to sanitize in dish machine. Must repair. Dish machine tagged and may not be used at time of inspection. Priority violation 7-38-025. Citation issued.
22. Proper cold holding temperatures
- comments: observed tcs foods at improper temperatures inside prep cooler in kitchen. Found block of ham @ 46. 1f; chicken @ 47. 2; diced tomatoes @ 47. 6; eggs @ 47. 6f; and sausage @ 48. 9f. Must keep all tcs foods at 41f or less at all times in refrigeration. Operator voluntarily discarded denatured foods during inspection. Citation issued. Priority violation 7-38-005.
37. Food properly labeled; original container
- comments: must provide common name label on all bulk items (flour, sugar, etc. ) in large containers in kitchen.
38. Insects, rodents, & animals not present
- comments: observe small gap along bottom left side of rear exterior door. Must repair to make all opening tightfitting to prevent pest entry.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chemical test kit on site to measure level of sanitizer in 3 compartment sink or dish machine. Must provide. Priority foundation violation 7-38-005. Citation issued.
49. Non-food/food contact surfaces clean
- comments: observed excessive grease accumulation inside lower interior of fryer. Must detail clean. Must maintain same.
51. Plumbing installed; proper backflow devices
- comments: observed slow draining 3 compartment sink in bar on 1st floor. Must repari and maintain same.
55. Physical facilities installed, maintained & clean
- comments: observed damaged paint/ceiling around pipe in liqour room at rooftop terrace. Must seal all holes and remove peeling paint.
58. Allergen training as required
- comments: must provide proof of allergen training for all chicago food sanitation managers.
60. Previous core violation corrected
- comments: observed previous core violations not corrected from report # 2286182, dated 5/2/19. Must correct the following previously issued core violations: #44 observed clean utensils and dished stored improperly on the shelf. Must invert all clean utensils and dishes during storage to protect from potential sources of contamination. #47 observed cardboard being used as shelf liner by the dishmachine in the kitchen. Must remove and provide shelf liner that is a non-absorbent, non-corrosive, smooth, easily cleanable surface. #49 observed the sides of the cooking equipment encrusted with an accumulation of food debris. Must remove debris, detail clean, and maintain. #53 - must provide covered trash receptacles in all women's toilet rooms. #55 - observed an extreme accumulation of debris and dirt encrusted to the 1st floor bar floor drain. Must remove debris, detail clean, and maintain at all times. Priority foundation violation
