- comments: observed in use ice cream spatulas stored within dipper well that was not turned on. Water was unclean. Must wash, rinse and sanitize dipper wells and spatulas and keep dipper wells running when in operation.

- comments: observed raw wood under underneath hot water heater. Raw wood is prohibited and is not smooth and easily cleanable. Raw wood needs to be cleaned in detail sealed so that it is non porous smooth and easily cleanable.

- comments: observed inadequate hot water of 67. 3 f at multiple sinks including hand washing sink, small 3 compartment sink and large 3 compartment sink. Must supply and maintain hot water of a temperature 100. 00 to 120. 0 f and a minimum temperature of 110. 0 f at 3 compartment sinks. Priority violation 7-38-030(c)

- comments: observed no back flow prevention device located on coffee machine. Must install a back flow prevention device on water line of coffee machine. Observed spliter for mop can not be placed after the vacuum breaker. Must remove spliter or install vacuum breaker after spliter.

- comments: observed floors unclean under and behind donut cases, under and behind refrigerators under and behind all dry storage shelving and under nad behind 3 compartment sinks. Clean in detail and maintain. Observed standing water in front of mop sink and hot water heater. Keep floor in a clean and dry condition.

- comments: observed non working refrigerator in north corner of donut area. Must remove all non working equipment and maintain area free from clutter.

60. Previous core violation corrected
- comments: observed previous core violations not corrected from report #2356057 dated 12/31/19: violation # 47 observed dark grooves in cutting board at both the dunkin donut prep area and the baskin robbins prep area. Must replace cutting boards. Violation #55 observed counter area in front of donut display within prep area unclean with excess food debris and crumbs. Must clean cracks and crevices and maintain area. Priority violation 7-42-090

- comments: we observed over 300 mice droppings throughout the premises. In selling area: under shelves, on top of shelves by the food (rice, flour, etc. ), on window ledges. We observed gnawed bags of dry foods by front widows and on shelves at selling area. Also observed a live mouse coming out running from under a shelf and went under two door reach-in freezer. Roodent droppings also in rear utensil washing area and storage: on floor, racks and equipment. Instructed to remove all droppings, sanitize and maintain. Have premises serviced by pest control control company, eliminate rodents. Priority violation:7-38-020(a) citation issued.
