I ate at the Hazlet Costco on July 27, around noon. I had their hotdog(relish, and ketchup), and churro. Those two items where the only things I had all day. Later that night around 7, I had diarrhea and my stomach felt upset.

My family and I ate USA salmon that we bought at Costco in Hazlet. Few hours later we got stomach ache, nauseas, vomiting, head ache.
