Date Closed: 02/25/2020High Priority - Employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing hands. Observed employee touching dirty utensils ( picking one of them from the floor), then touching clean utensils without handwashing. Employee was coached about the procedure and he washed his hands after it. **Corrective Action Taken**
High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed approximately 5 plus flies on the backside of the kitchen area.
High Priority - Raw animal food stored over ready-to-eat food in walk-in cooler. Observed raw chicken above Alfredo fettuccine. Manager inverted the food storage. **Corrected On-Site** **Repeat Violation**
High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 25 plus live roaches inside reach in freezer gaskets, 15 plus live roaches crawling on the floor behind the equipments, 4 plus live roaches crawling by the preparation table next to steam table, and 3 plus live roaches inside unused equipment next to steam table, all in the kitchen .
High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Kitchen area ( backside, near exit door)
