a) 3 live adult cockroaches on the floor next to the ice machine in the food preparation area.
b) 1 live adult cockroach on the floor next to the upright beverage cooler located in the food preparation area.
c) 1 egg capsule on the floor next to the ice machine in the food preparation area.
d) 10+ dead adult and nymph cockroaches on the floor behind the ice machine.
