- comments: observed live fruit flies in the rear 3-compartment sink area and cluttered storage areas. Approximately 25 live fruit flies were observed on the walls, ceiling and equipment in these areas. Instructed to clean and sanitize affected areas and additional pest control service is needed to minimize or eliminate the pest activity. Priority foundation violation 7-38-020(a).

- comments: observed missing sink stoppers for the 3-compartment sink area. Instructed to provide 3 adequate working sink stoppers and to maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle provided in washroom. Management instructed to provide.
55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean, organize rear storage areas and remove all unnecessary or unused articles and equipments due to potential pest harborage and items kept must be properly stored and elevated at least 6' inches off the floor for easier cleaning.

- comments: floors along walls and in all corners in rear 3 compartment sink area and storage areas and in walk-in-cooler in need for detail cleaning with debris also,walls in the rear 3 compartment sink area need detail cleaning to remove food splashed debris and to maintain.
