- comments: observed no vomit and diarrhea clean up kit/procedure on site. Must provide written procedure for the clean up of vomit and diarrheal events, and corresponding equipment outline in procedure. Priority foundation violation 7-38-005. No citation issued.
36. Thermometers provided & accurate
- comments: observed no thermometer inside reach in coolers. Must provide a thermometer in a conspicuous spot in all refrigeration units. Must maintain same.
38. Insects, rodents, & animals not present
- comments: observed approximately 20 small black flies on walls and in area around 3 compartment sink in rear of store. Must eliminate pest issue. Must clean and sanitize the area. Priority foundation violation 7-38-020(a). No citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed large cracks, damaged and missing tiles in floor in rear of store. Must repair and make floors smooth and easily cleanable. Must maintain same. Observed clean water in rear area of basement floor. Must keep clean and dry.
61. Summary report displayed and visible to the public
- comments: observed no previous inspection report summary posted in facility. Instructed must post most recent summary in plain view of customers at all times. Operator given summary to post during inspection. Priority foundation violation 7-42-010(b). No citation issued.
62. Compliance with clean indoor air ordinance
- comments: observed ashtray with cigarette butts inside. Instructed must remove all ashtrays from premises. Instructed smoking not permitted inside any chicago business. Operator corrected during inspection. Priority foundation violation. No citation issued. 7-32-020.
