- comments: - must provide handwash signage at all handwash sinks and maintain. -

- comments: - must provide a thermometer inside the hot hold unit and maintain. -

- comments: - must install a splash guard between the rear handwash sink and the clean dish storage bin. -

- comments: - observed dirty rag stored on the splash guard of the front handwash sink. Must remove and air dry all rags in a clean dry place, away from potential sources of contamination. -

- comments: - unable to observe the backflow prevention device on the mop sink. Must be able to demonstrate location of the mop sink's backflow prevetion device, for visible inspection. -

- comments: - observed dirt particles and cloudy waste water leaking from the grease trap onto the floor in the rear prep area, when the rear 3 compartment sink is drained. Observed dirt particles and cloudy waste water backing up into the other sink compartments when water is run into one of the compartments of the front 3 compartment sink. Must repair sinks to adequate convey sewage. Priority violation#:7-38-030(c). Citation issued. -

- comments: - observed the mop stored inside the dirty mop bucket. Observed the brooms and other cleaning tools stored directly on the floor in the utility closet. Must remove and store all mops, broom, and other cleaning apparatuses off the floor and in a clean dry location, away from potential sources of contamination. -

- comments: - observed a hole with water damage in the ceiling in the utility closet. Must repair and maintain. -

56. Adequate ventilation & lighting; designated areas used
- comments: - observed slight debris/grease buildup on the kitchen exhaust ventilation hood. Must detail clean and maintain. -

- comments: - observed a new certified manager on site does not have the allergen awareness training certificate. Must provide allergen training certificate for all certified managers and maintain. -
