Went to E.R. Have copies of reipt & Hospital diagnosis. Vomited & diarrhea for 2 days & nights before going to
Trumbul l Memorial Hospital, Warren, Oh. 44483. Got Violently I'll about 6 hours after eating Southwest Salad @ Wendy's. 422 Niles, 44446. The only thing I ate that A.M. was a cup of coffee & a bagel. Following the stomach illness, I had sores in my mouth, under tongue. Currently in hospital with extremely bad kidney infection that could become Sepsis or kidney failure. This incident occurred on April 7th.
