ER Visit After Salad - Wendy's, Niles Ohio

Went to E.R. Have copies of reipt & Hospital diagnosis. Vomited & diarrhea for 2 days & nights before going to
Trumbul l Memorial Hospital, Warren, Oh. 44483. Got Violently I'll about 6 hours after eating Southwest Salad @ Wendy's. 422 Niles, 44446. The only thing I ate that A.M. was a cup of coffee & a bagel. Following the stomach illness, I had sores in my mouth, under tongue. Currently in hospital with extremely bad kidney infection that could become Sepsis or kidney failure. This incident occurred on April 7th. It's the 28th and I'm still very Ill. | Symptoms: Diarrhea, Fever, Nausea, Vomiting, Soreness
