- comments: observed no person in charge with the original city of chicago food manager certificate. Instructed to obtain and post the city of chicago food manager certificate on site at all times. Priority foundation violation. 7-38-012. Citation issued.

- comments: observed no soap and paper towels provided at the hand washing sink at front bar area. Instructed to stock soap and paper towels at hand sink at all times to properly wash and dry hands. Priority foundation violation. 7-38-030(c).

- comments: observed ice scoop stored inside of ice bin at bar area. Must store ice scoop in holder.

- comments: observed chipped ice scoops at ice bin in basement. Instructed to replace and provide ice scoops that are smooth and easy to clean.

- comments: observed heavy grease build up inside of cooking fryer in kitchen prep area where speciality food items are being prepared. Must clean and maintain.

- comments: observed excessive clutter in rear hallway leading to outside dumspter with unused grill, card board boxes and food cart with dirty dishes. Must remove and clean area to prevent pest harborage.

- comments: observed no allergen training certificates on site. Instructed to provide allergen training for all employees who obtain the city of chicago food manager certificate.
