5. Procedures for responding to vomiting and diarrheal events
- comments: found missing items for vomit spill kit such as face masks and sanitizer rated for norovirus. Priority foundation. No citation issued. 7-38-005. Must provide all supplies.

10. Adequate handwashing sinks properly supplied and accessible
- comments: found no handwashing signage at all hand sinks. Must provide.

37. Food properly labeled; original container
- comments: found bulk food containers with no labels: flour, spices, ingredients etc. Must provide.

49. Non-food/food contact surfaces clean
- comments: found cooking equipment and piping behind cooking equipment not clean at 1st floor main kitchen, accumulated grease and food debris. Must clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: found no backflow prevention devices at following areas: for hose at dishmachine area lower level, hose by 3 compartment sink lower level area, and hose at dry storage area. Also, air gap required for drainage tubing at ice machine. Must provided back flow prevention devices where they can be serviced/located.

51. Plumbing installed; proper backflow devices
- comments: found leaky drainage piping at 3 compartment sink at 1st floor bar. Found leaky drainage piping at lower level dishmachine area hand sink and dirty liquid discharging from pipe under left side of dishmachine in corner. Must repair and maintain plumbing.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: found trash cans inside women's and unisex restrooms with no covers. Must provide.

55. Physical facilities installed, maintained & clean
- comments: found floors, walls and ceiling not smooth and easy to clean at following areas: wall behind bar 1st floor (exposed brick), by rear stairwell behind main kitchen (chipping paint), lower level floor worn grouting in dishwashing and food prep areas, ceiling right side of stoves at lower level, and ceiling at dry storage areas in basement. Must repair.

55. Physical facilities installed, maintained & clean
- comments: found unused ice bin at bar area 2nd floor. Must remove.

56. Adequate ventilation & lighting; designated areas used
- comments: found exhaust vent not working inside handicap restroom. Must repair.

57. All food employees have food handler training
- comments: found no foodhandler training certificates. Must provide.

58. Allergen training as required
- comments: food sanitation managers with no allergen training. Must provide.
