Report date: Apr 16, 20205. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 125 or more mice droppings on floor under and along wall bases all display shelves on sales floor,on display shelves behind service counter and candy,party supplies,baby essential,along wall bases in rear storage areas. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/16/20. (priority foundation 7-38-020(a)

- comments: observed an appx. "1/2-3/4" gap in center bottom of double front door. Must make door tight fitting.
49. Non-food/food contact surfaces clean
- comments: must clean debris build up in reach in coolers and freezers,display shelves,mop sink basin,window sills and where needed.
51. Plumbing installed; proper backflow devices
- comments: must provide a backflow device on mop sink.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide a garbage can with lid in women's toilet room.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside garbage dumpster overflowing with trash and lids wide open. Must have lid closed and trash below rim and tight fitting. (priority foundation 7-38-020(b)

- comments: must detail clean excessive debris and build up on floor under and around all display shelves on sales floor and service counter.
