- comments: 6-501. 111 observed evidence of live insect activity. Observed about 5 large flies flying in the food prep area, lunchroom area and in the toilet room. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation 7-38-020(a).

- comments: 5-205. 15 observed the hot water knob missing from the rear hand washing sink located next to the 3 compartment sink. Instructed to repair.
