- comments: observed tape on the reach-in cooler door where the white castle burgers are stored. Instructed to remove and repair.

- comments: observed the interior of display freezer where frozen bag of vegetables are storerd in eed of cleaning. Instructed to clean.

- comments: observed the left hand sink in the women restroom to have a slow drain. Instructed to ight hand sink battery to faucet in need of changing. Instructed to repair.

55. Physical facilities installed, maintained & clean
- comments: observed a three holes in the wall in the staff room above the exposed sink. Instructed to seal.
