Possibilities include in the restaurant, which is an inspection item, from storage near other food without covering, refrigeration temps above 41 degrees; handling of various foods without changing gloves or washing hands or having no soap at handwashing sink; or produce contaminated "in the field" which could include manure, or again, dirty handling; or fecal bacteria from chicken getting around even a cleaned kitchen. I found a source that some of the bacteria were bleach-resistant. Some restaurant inspections show too low a concentration of bleach in dishwashing.
