- comments: noted no certified food manager on duty while tcs foods were prepared and served at the time of inspection. Certified food manager arrived at facility at 12:15 pm. Informed facility a original food service certificate must remain posted on site and a certified manager must be on premises at all times when time and temperature control for safety(tcs) foods are being prepared, handled and/or served. Priority foundation violation 7-38-012. Citation issued.

- comments: observed a black slime like substance on the interior panel of ice machine, ice likely for human consumption. Instructed to discard ice and have machine service, cleaned and sanitized, priority foundation 7-38-005 citation issued

- comments: must label food storage containers with common food names; flour, sugar, etc. , when food has been removed from original packaging. Instructed to maintain.

- comments: found approximately 15 live small on the walls in the rear food prep area. Instructed to remove flies, clean and sanitize affected area. Contact pest control for service and maintain. Priority foundation 7-38-020(a) citation issued.

- comments: found wet wiping cloth stored on various surface areas in the front coffee/bar prep area and rear prep. Must properly store wiping cloths in a sanitizing solution between use.

- comments: found vlade on mounted can opener in rear prep not clean. Cutting or piercing parts of can openers shall be readily removable for cleaning and for replacement. Instructed to clean and maintain.
