Informed facility a original food service certificate must remain posted on site and a certified manager must be on premises at all times when time and temperature control for safety(tcs) foods are being prepared, handled and/or served. Priority foundation violation 7-38-012. Citation issued.

- comments: observed a black slime like substance on the interior panel of ice machine, ice likely for human consumption. Instructed to discard ice and have machine service, cleaned and sanitized, priority foundation 7-38-005 citation issued

- comments: must label food storage containers with common food names; flour, sugar, etc. , when food has been removed from original packaging. Instructed to maintain.
