- comments: 6-301. 14 observed a hand washing sign is needed in the toilet room. Instructed to provide.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of rodent and live insect activity. Observed about 50 rodent droppings scattered thru out the dry storage shelves located in the customer service area, about 10 rodent droppings located at the bottom shelf of the prep table located in the deli prep area and about 15 rodent droppings on the floor located betweem the prep table and the reach in freezer also located in the deli prep area. Observed about 3-5 live large flies flying in the facility. Instructed to eliminate all pest activity and clean and sanitize affected areas. Priority foundation violation. Citation issued 7-38-020(a).

- comments: 5-501. 15 observed the outside dumpster ground area not maintained with excessive garbage on the ground. Instructed to clean and maintain outside dumpster area at all times. Priority foundation. Citation issued 7-38-020(b).

- comments: 6-501. 12 observed floors and walls through out premises with accumulated debris. Instructed to clean floors and walls thru out the entire facility.
