- comments: observed no city of chicago certified food manager on site while tcs foods are being prepared/cooked (such as pizza, wings, etc. ). Instructed must have a city of chicago certified food manager on site when tcs foods are being prepped, cooked and/or served. Observed no city of chicago food sanitation manger certificate posted in front of facility. Instructed must post a valid city of chicago food sanitation manager certificate in plain view of customers at all times. Priority foundation violation 7-38-012. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit and diarrhea clean up kit/procedure on site. Must provide written procedure for the clean up of vomit and diarrheal events, and corresponding equipment outline in procedure. Priority foundation violation 7-38-005. No citation issued.

- comments: observed no paper towels in dispensers at hand sinks/ washbowl sink. Instructed must have sanitary means to dry hands at all hand sinks, washbowl sinks. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed no dates on prepped tcs foods inside coolers and walk in. Must properly label all prepped tcs foods with discard date no more than 7 day from preparation date. Must correct and maintain same. Priority foundation violation 7-38-005. No citation issued.

- comments: must provide stoppers for 3 compartment sinks.

- comments: observed waste water not draining in service sink on 2nd floor. Must repair service sink to drain all waste water. Priority violation 7-38-030(c). Citation issued.

- comments: observed excessive clutter and debris on floors throughout 2nd floor. Must elevate all items 6 inches from the floor and 6 inches from the walls to allow for proper cleaning. Must organize all clutter and also remove any equipment not in use to prevent pest harboage.

Observed appx 50 small, black flies on walls and ceilings near 3 compartment sink and in rear of prep area near mop sink. Instructed must detail clean and sanitize these areas. Must contact pest control for service to minimize pest issue. Priority foundation violation 7-38-020(a). No citation issued. Observed approximately 25 mice droppings on floors on 2nd floor. Must remove, clean and sanitize. Priority violation 7-42-090. Citation issued.
